| Making an Appointment
Appointments to see any of our clinic physicians may be scheduled
by calling the office location where that doctor
practices. All
primary care physicians and most specialists see patients without
doctor referral. Because we are a multispecialty clinic, numerous
surgical
and medical specialties are represented. Your confidential medical
record is available to your clinic physicians regardless of which
location you visit.
If your insurance plan requires a referral form, the primary care
physician will handle the paper work. Please remind us of the insurance
plan in which you participate.
Pre-appointment Form
For your convenience, we provide a printable
pre-appointment form (pdf format). Once the form is displayed,
print it from your web browser, complete the information, and bring
the form with you to the clinic to speed up the process of checking
in.
Insurance
NEA Clinic's insurance department files each patient's claims with
their primary and secondary insurance carriers. This includes all
commercial carriers as well as Medicare and Medicaid. Patients are
requested to assist by bringing all insurance information at the
time of their first visit. If there is only one insurance carrier,
clinic policy requires payment of 20% at that time, plus any deductible
not met for the year. If there is no insurance coverage, payment
in full is customary, unless other arrangements have been made prior
to the clinic visit.
Accounts
Financial responsibility for medical services lies with the patient
and not an insurance company or government agency. Patients are
expected to keep their accounts current.
Fees
Fees will vary for various conditions, but generally reflect the
specialized ability and time devoted to each problem. Return visits
require less extensive evaluation and time with the doctor; accordingly,
fees are lower. Our policy is open disclosure of fees to our patients
at any time upon request.
Privacy Practices
If
you are a first-time patient or a returning patient who has not yet
signed our privacy practices notification, you can save yourself
a bit of time on paperwork when visiting our office by printing our
privacy notice now and bringing the signed document with you on the
day of your appointment. This notice describes how medical information
about you may be used and disclosed and how you can get access to
